| An examiner from APPS will promptly contact your client to schedule an appointment at a time
and place convenient to him or her. At that time, the examiner will discuss exactly what procedures
will be performed. The following are some of the most frequently required: |

| PARAMEDICAL REPORT |
| (15 Minutes) A short questionnaire asking for a complete medical history,
vital signs (blood pressure, pulse, height and weight) and a urine sample. |
| BLOOD PROFILE |
| (10 Minutes) Blood will be drawn from the arm or finger as required by your home office. A urine sample
will also be collected. Only sterile, disposable needles and supplies are used. The specimens are then
sent to a laboratory designated by your home office. The results are sent directly from the lab to underwriting.
APPS never learns of any test results. |
| ELECTROCARDIOGRAM/ EKG |
| (10 Minutes) An EKG is a recording of the electrical impulses associated with the heart's
contraction and relaxation. An EKG is a painless procedure in which electrodes are placed on the chest,
upper arms and lower legs. To achieve an accurate reading, the client lies flat in a relaxed
position for the duration of the exam. Privacy is required since your client must partially disrobe. |
| PHYSICIAN'S EXAM |

| (25 Minutes) In addition to the elements in the Paramedical Report, our doctor will also
perform a brief physical which involves listening to the client's heart and lungs as well as
other medical checks. |
| TIME VITAL CAPACITY TEST/ TVC |
|
| (5 Minutes) This test measures lung capacity. It requires your client to blow into a disposable
cardboard tube. |
| TREADMILL/EKG |
| (60 Minutes) An EKG taken while your client exercises. An appointment will be scheduled at a
medical facility near your client since this is not a mobile test. |
| CHEST X-RAY |
| (20 Minutes) APPS has mobile x-ray units in many locations. For clients outside these areas,
we will schedule an appointment at a convenient medical facility. |
